Sealed tenders are invited from reputed firms having registration with the Sales Tax and Income Tax Departments for the provision of following benefits to be covered under medical insurance service.
Instructions: Medical Insurance period will be from 1st July, 2008 till 30th June, 2009.
First quotation with medical insurance of Employee, Spouse & Dependents.
Second quotation with medical insurance of Employee, Spouse, Dependents & Parents.
Further classification & detail regarding respective medical limits, number of employees, age etc can be obtained from the office of the undersigned after registration upon payment of Rs. 500/- in the form Demand Draft / Pay Order in the name Universal Service Fund. (Registration form can be downloaded from the company’s website).
Last date for submission of the tender document at the address given below will be 07-07-2008 at 12:00 PM and will be opened on the same day at 12:30 PM in the presence of bidders and/or their authorized representatives at the address given below. The advertisement can be found on PPRA website (http://www.ppra.org.pk/) and on USF website (http://www.usf.org.pk/).
USF Company has the right to reject all bids or proposals at any time prior to acceptance of bid or proposal.
