The Office of Executive District Officer Health District Government Tharparkar at Mithi is inviting applications from Manufacturers / Importers for pre-qualification in the category “For Supply of Drugs / Medicines / Disposable / Surgical Items, X-Ray Films/ Chemicals, Furniture / Fixture, Machinery & Equipments for the Year 2008-09”.
The interested firms are advised to obtain the request Form from the office of undersigned, Additional Secretary (PM&I Cell) Health Department, Block-24, Shahrah-e-Liaquat Karachi and the Director General Health Services Sindh Hyderabad, on the cash payment of Rs.100/- (One hundred rupees) (Non-refundable) up to 20th October 2008 and the same form along with full required information will be deposited in the office of undersigned up to 25.10.2008. An amount of Rs.1000/- will be charged as service charges only from qualified Manufacturer/ Importer.
Note: Those firms who are pre-qualified for the year 2006-07 and 2007-08 will apply for renewal for pre-qualification during the year 2008-09 along with Pay Order Rs.1000/- for service charges in favour of undersigned and relevant documents. The list of qualified firms for the year 2006-07 & 2007-08 is also available in the office of Additional Secretary Procurement (PM&I Cell) and Directorate General Health Services Sindh Hyderabad.
