Sealed tenders are invited on the prescribed forms on Two Envelopes Single stage procedure as per Public Procurement Rules 2004 from the Manufacturers/ Importers/ Sole Agents/ Contractors for the purchase of following categories for the Schemes of Health Institutions/Health Facilities of CDGK for the year 2008-09.
(1) ADP (Annual Development Program) Schemes.
(2) SDSSP (Sindh Devolved Social Service Program).
(3) Major Hospitals of defunct KMC.
(4) Health Facilities of defunct DMCs/DCK/KDA.
(5) Health Institutions/Health Facilities of devolved GOS.
(6) Karachi Institute of Heart Diseases CDGK.
Note: - Quantities and Specifications of each items are mentioned in the tender documents provided to the intending bidders.
Prescribed tender proforma along with copy of terms and conditions can be obtained on the payment, mentioned in the above schedule against pay order (Non Refundable) in the favour of EDO Health, CDGK & on production of following documents from the office of EDO (Health) CDGK 6th Floor Civic Centre, Karachi (ii) EDO Finance & Planning CDGK 6th Floor Civic Centre, Karachi and (iii) District Officer Medical Services near Islamia College Karachi on any working day.
(1) Latest Income Tax Certificate
(2) GST Registration Certificate
(3) Computerized NIC of the authorized person.
The bidding shall be on "Single Stage-two envelope procedure: -
(1) 1st Envelope should contain Technical Proposal only and be clearly marked "Technical Proposal"
(2) 2nd Envelope should contain Financial Proposal (Rates) (Separate envelope for each quoted Item) along with Earnest money @ 2% of the quoted rates in the shape of pay order / demand draft (refundable to unsuccessful bidders) the envelop be clearly marked as 'Financial Proposal"
Clarification: The rate of Each item should be quoted separately and along with earnest money be sealed in a separate envelope and labelled as Financial Proposal . Name of item and name of the firms/company should be written in bold letter on the envelope. Other wise it will not be entertained.
Both Envelopes (Technical and Financial) should be sealed separately and placed in a 3rd Envelope. The name of the bidder be clearly marked on all envelopes.
Tender should be dropped in the tender box kept in the office of the EDO (Health) CDGK @ 6th Floor Civic Centre, Karachi mentioned above before 01.30 p.m. on the schedule dates.
The Technical Proposals of bids shall be opened publicly at 02.00 p.m. on the same day in presence of bidders or their nominated representatives who wish to attend.
After the evaluation and approval of the technical proposals of medicine and allied on the basis of documents/samples and approval of technical proposals of equipments / instruments on the basis of samples /demonstration/ Original Brochures, the CDGK shall at a time within the bid validity for minimum of 60 days, publicly open the financial proposals of the technically accepted bids only. The financial proposal of bids found technically non-responsive shall be returned un-opened to the respective bidders.
The Purchase committee may cancel/delete any item or decrease or increase the quantity or order for part supply part payment as per requirement. Tenders who do not fulfill the terms & conditions will not be entertained. CDGK reserves the right to accept or reject any one or all tenders.
The Government notified black listed firms/suppliers shall not be entertained.
In case the tender is not opened on schedule dates due to any force de majure then the same shall be opened on next working day.
Note: - A pre-bid meeting with Manufacturers/ Importers/ Sole Agents/Contractors and suppliers who are interested to participate in the tender, is scheduled on 22nd September 2008 at 11.00 a.m. to discuss the procedures and matters related to the procurement. All interested parties are invited to attend the meeting.
